Construção e validação da escala de suporte formal para a autonomia na dor nas pessoas idosas

Abstract

Apesar de, na literatura, estar descrito o beneficio de um suporte social percebido como promotor de autonomia na funcionalidade física associada à dor, não se encontraram instrumentos que reconheçam a função de promoção/supressão de autonomia no âmbito da percepção de suporte social, no contexto institucional em pessoas idosas com dor. Assim, este trabalho visou construir e validar um instrumento que meça a percepção de suporte social formal face à promoção e supressão da autonomia das pessoas idosas na sua vivência com dor – a Escala de Suporte Formal para a Autonomia na Dor (ESFAD). Participaram neste estudo 151 idosos (88,1% mulheres) entre os 56 e os 94 anos (M=75,41; DP=9,11) que frequentavam três tipos de instituições: Centro de Dia (33,1%), Lar (36,4%) e Universidade Sénior (30,5%). Foi pedido aos participantes que preenchessem a ESFAD juntamente com outros instrumentos utilizados no processo de validação da escala tais como o Inventário Resumido da Dor (Cleeland, 1989), a escala de Suporte Social do Medical Outcomes Survey (Sherboune & Stewart, 1991) e algumas questões sobre as suas experiências passadas e actuais de dor. Quanto aos resultados, a análise da estrutura factorial da ESFAD revelou as duas dimensões teoricamente definidas pelas funções de supressão e promoção da autonomia, que apresentaram valores adequados de fidelidade interna e de sensibilidade. Quanto à validade da ESFAD, verificou-se que os seus itens apresentam uma boa validade de conteúdo. Ainda, é um instrumento que apresenta uma boa validade relativa a um critério dado que permite diferenciar as percepções de supressão e promoção de autonomia em função do: 1) tipo de dor actual, sendo que os idosos que sofrem de dor crónica percebem mais promoção de autonomia; 2) dos aspectos qualitativos da dor, verificando-se que entre os idosos com dor crónica quanto maior a severidade e grau de interferência da dor maiores são as percepções de supressão da autonomia e 3) tipo de instituição, verificando-se que as percepções de promoção e supressão da autonomia foram mais elevadas nas instituições cujos utentes possuem mais condicionantes ao nível de autonomia – no Centro de Dia e no Lar. Finalmente, a ESFAD apresenta uma boa validade discriminante, não se correlacionando significativamente com a Escala de Suporte Social do Medical Outcomes Survey. Assim, conclui-se que a ESFAD é um instrumento com boas qualidades psicométricas, que contribui para a colmatação da inexistência de um instrumento de avaliação das percepções de suporte social formal das pessoas idosas com dor, dando centralidade à função de promoção da autonomia enquanto determinante de um processo de envelhecimento activo e bem sucedido.

Although, in the literature there are references to the benefits associated with perception of social support as a promoter of autonomy with respect to functionality associated with physical pain, there were no instruments that emphasize the promotion/suppression of autonomy as a qualitative function of the perception of formal support social, in the context of older people with pain. This study aimed to construct and validate an instrument that measures the perception of autonomy promotion and suppression of older adults, resulting from the formal social support, in their experience with pain – Pain Autonomy Formal Support Scale (ESFAD). 151 seniors participated in this study (88.1% women) between 56 and 94 years (M=75.41, SD=9.11) who attended three types of institutions: Day Centre (33.1%), Home (36.4%) and Senior University (30.5%). Participants were asked to complete the ESFAD along with other instruments used in the scale validation process, such as the Brief Pain Inventory (Cleeland, 1989), the Social Support Scale of the Medical Outcomes Survey (Sherboune & Stewart, 1991) and some questions about their past and current experiences of pain. The results analysis revealed that the factorial structure is built of the two dimensions theoretically defined by the functions of autonomy suppression and promotion, which showed appropriate values of internal consistency and sensitivity. It was found that ESFAD items have good content validity. It is an instrument that presents a good validity on a given criteria which allows the distinction of the autonomy suppression and promotion perceptions concerning the: 1) type of current pain, reflecting that the elderly who suffer from chronic pain often experience increased promotion of autonomy, 2) qualitative aspects of pain, verifying that in the elderly suffering with chronic pain the greater the degree of severity and pain interference the greater are the autonomy suppression perceptions and 3) type of institution, verifying that the perceptions of promotion and suppression of autonomy was highest in institutions whose clients have more constraints to the level of autonomy - in the Day Center and Nursing Home. Finally, ESFAD shows good discriminant validity, showing no significant correlation with Social Support Scale from the Medical Outcomes Survey. Thus we can conclude that ESFAD is an instrument with good psychometric qualities, which contributes to the clogging of the lack of an instrument to assess perceptions of formal social support of older people with pain, giving the central role of autonomy support as a determinant of an active and successful aging.
