Alta hospitalar, serviço social e direitos humanos

Abstract

A presente investigação teve como objetivo identificar e analisar a prática do assistente social em contexto de planeamento de alta hospitalar, na promoção dos Direitos Humanos, bem como, a identificação dos determinantes que possam ser fatores condicionantes das altas com protelamento social. Neste contexto utilizamos uma metodologia essencialmente assente no modelo indutivo, englobando uma abordagem mista, que integra a análise quantitativa dos casos dos utentes protelados do CHLC na relação com a análise qualitativa no que concerne à perspetiva dos profissionais e respetiva coordenação do Serviço Social. Constatámos que para além das condições de saúde dos utentes, o grau de envolvimento da família em todo o processo, bem como a sua capacidade de apoio ao utente implicam significativamente o nível de complexidade da intervenção. Bem como as situações mais complexas implicam internamentos mais prolongados em comparação com as de baixa complexidade. O protelamento da alta hospitalar é condicionado por diversos fatores, extrapolando os de caráter exclusivamente social, sendo que numa primeira instância é determinado pelo contexto de dependência do utente. Na sua prática, os assistentes sociais regem-se pela defesa dos Direitos Humanos, traduzindo-se em tarefas e atividades concretas, nomeadamente no planeamento da alta hospitalar, sendo que o seu protelamento, pode funcionar como um instrumento de manutenção ou inibição dos mesmos em conformidade com a gestão da situação, e com o espaço de tempo até à efetivação da alta hospitalar.

The objective of the present investigation was to identify and analyze the practice of social worker in the context of hospital discharge planning, in the promotion of Human Rights, as well as the identification of the determinants that may be factors determining the discharge with social delay. In this context, we use a methodology based essentially on the inductive model, encompassing a mixed approach, which integrates the quantitative analysis of the cases of patients with delayed discharges of CHLC and a qualitative analysis with the perspective of professionals and the coordinator of Social Work. It was found that in addition to the health conditions of the patients, the degree of involvement of the family in the whole process, and their capacity to support the patients, significantly implies the level of complexity of the intervention. It has also been found that more complex situations involve longer hospitalizations compared to those with low complexity. In this sense, delayed hospital discharge is conditioned by several factors, extrapolating those of exclusively social character, so much that in a first instance it is determined by the context of incapacity of the patient. In their practice, social workers fight for the defense of Human Rights, which translate into concrete tasks and activities, particularly in the planning of hospital discharge, and their delayed can function as an instrument of maintenance or inhibition of the Human Rights in conformity with the management of the situation, and the time until the hospital discharge.
