O efeito da coesão e flexibilidade familiar e a influência do apoio social na pessoa com doença mental grave

Abstract

O desenvolvimento de novos psicofármacos, nos anos 60, permitiu a desinstitucionalização de pessoas com doença mentalgrave e o seu retorno à comunidade. Alguns estudos documentaram o aumento de recaídas em pessoas com doença mental grave aquando inseridas na sua família, comparativamente a pessoas a viverem sozinhas. Têm sido estudadas variáveis familiares que influenciam estas pessoas. O papel do apoio social tem sido amplamente estudado e documentado o seu efeito positivo na saúde mental. Este estudo, pretendendo contribuir para o desenvolvimento deste conhecimento, teve como objectivo conhecer o impacto da coesão e flexibilidade familiar na autonomia no funcionamento geral, ocorrência de sintomatologia, satisfação com a vida e qualidade de vida da pessoa com doença mental grave. E ainda conhecer o papel moderador do apoio social nesta relação. Participaram no estudo 123 pessoas adultas, entre os 20 e 77 anos, em número semelhante de homens e mulheres, com doença mental grave e com capacidade de compreensão e responder ao questionário de recolha de dados. Cerca de 67% referiram viver com a família, os restantes referiram viver ou com colegas ou sozinhos. Foi feita uma análise estatística multivariada através da regressão linear múltipla. Conclui-se que maior coesão familiar estava associada a maior autonomia no funcionamentogeral. A flexibilidade familiar não esteve associada a nenhuma variável de critériodo modelo conceptual. O apoio social não teve papel moderador, no entanto mostrouser a variável com maior influência directa em todas as variáveis de critério em estudo.

The development of new psychiatric drugs, in the 60s, allowed the deinstitutionalization of people with serious mental illness and their return to the community. Some studies have documented an increase in relapses in people with severe mental illness when inserted in their families, when compared with people living alone. Have been studied family variables that influence these people. The role of social support has been widely studied and documented its positive effect on mental health. This study, intending to contribute to the development of this knowledge, aimed to understand the impact of family cohesion and flexibility in the autonomy in general functioning, occurrence of symptoms, life satisfaction and quality of life of people with severe mental illness. And know the moderating role of social support in this relation. Participated in the study 123 adults, between 20 and 77 years, similar number of men and women with serious mental illness and ability to understand and respond to the questionnaire data collection. About 67% reported living with family; the rest reported living either alone or with colleagues. We conducted a multivariate statistical analysis bymultiple linear regression. We conclude that greater family cohesion was associated with greater autonomy in general functioning. Flexibility family was not associated with any criterion variable of the conceptual model. Social support hadn’t a moderating role, however proved to be the variable with the greatest direct influence on all criterion variables under study.
