A variabilidade da prática médica: uma abordagem em radioterapia

Abstract

O cancro constitui um importante problema de saúde pública das sociedades modernas sendo uma das principais causas de morbilidade e mortalidade, (segunda causa de morte na União Europeia em 2006). O tratamento da doença por radioterapia envolve uma componente tecnológica muito diferenciada. Em saúde o progresso tecnológico é constante e avassalador, o que conduziu a uma evolução dos tratamentos e consequentemente à adaptação da prática médica. Esta evolução tecnológica proporciona diferentes abordagens, que podem induzir um maior consumo de recursos se não houver protocolos bem definidos. A variação da prática médica, VPM, não está muito estudada em oncologia e em Portugal não foram encontrados estudos nesta área. Foi a constatação de que esta é uma área que importa aprofundar para melhor conhecer e agir, o que nos levou ao desenvolvimento deste projeto em que procuramos “Comparar a prática médica em radioterapia nos procedimentos do tratamento identificando a sua variabilidade em diferentes Instituições”. Este é um estudo exploratório e descritivo. A população é constituída por doentes oncológicos tratados por radioterapia nas patologias da próstata, mama e ginecologia em quatro instituições. A amostra é constituída por 360 doentes, 30 doentes por patologia e por instituição. Como principais resultados encontramos a variabilidade nos tratamentos, nos seus procedimentos e custos entre instituições. Destacam-se nos custos de tratamento, o Hospital 1 com valor mais elevado para o tratamento da próstata, mama e ginecologia com braquiterapia, e o Hospital 4 com o valor mais baixo. Realça-se a diferença significativa nos custos médios de tratamento da mama e ginecologia entre hospitais, não se verificando de forma tão considerável para a próstata.

Cancer is an important public health problem of modern societies being one of the main causes of morbidity and mortality (second leading cause of death, in European Union in 2006). The cancer patient consumes a lot of means in the treatment of the disease with radiation therapy involving a differentiated technological component. The technological progress in the health service is constant and overwhelming, and consequently in Radiation Therapy, which has led to the advance of treatments and to the adjustment of the medical practice. This technological evolution provides different approaches, which in case well defined protocols do not exist, can lead to a higher consumption of means. The medical practice variation, MPV, is not extensively studied and in Portugal there are no studies in this area. It was the realization that this is an area worth investigating in order to know and act better which led us to the development of this research project in which we try to (compare) “Comparing medical practice in radiation therapy within the treatment procedures identifying its variability in different institutions.” It is an explorative and descriptive study. The population is cancer patients treated with radiation therapy in the following pathologies: prostate, breast and gynecology. The sample is 360 patients. Thirty patients being observed for pathology, and for institution. As main results, the existence of the variability in the treatments, in their procedures and the costs of the different pathologies. The costs of treatment stand out, Hospital 1 has the highest value for the treatment of the breast, for gynecology with brachytherapy and prostate, and Hospital 4, the lowest value. The significant difference in the average treatment costs for the breast and gynecology between hospitals is emphasized; the difference for the prostate is not so considerable. In the analysis of the various procedures we still find the existence of variability between institutions though not varying between each other in the same way.
