Acesso à saúde por imigrantes com infeção VIH em Portugal

Abstract

O crescente fluxo migratório e o aumento de novos diagnósticos de VIH nos imigrantes em Portugal fazem com que o tema do acesso à saúde desta população seja cada vez mais pertinente, com implicações na saúde dos próprios e na saúde pública. Apesar de a nossa legislação promover o acesso universal à saúde, este direito sofre influência de muitos outros fatores (determinantes) que importa conhecer para promover o desenvolvimento de políticas e práticas verdadeiramente integrativas. Com este objetivo foi realizada a presente pesquisa que utilizou uma metodologia mista de investigação, qualitativa e quantitativa: 1) grupos focais (com profissionais de saúde e colaboradores de ONG), e 2) inquérito por questionário, aplicado a nível nacional a 407 imigrantes com VIH residentes em Portugal. Os resultados encontrados permitiram identificar uma maior vulnerabilidade dos imigrantes com VIH face aos nacionais, revelando que mais de metade já experienciaram diversos obstáculos no acesso e utilização dos serviços de saúde. Os determinantes identificados coincidem em grande parte com os referidos na literatura para os imigrantes em geral, incluindo fatores inerentes aos indivíduos, aos profissionais de saúde e sua relação com os utentes, e questões estruturais do SNS. Verificou-se ainda que os fatores que influenciam o acesso e utilização dos serviços gerais de saúde não são totalmente coincidentes com os relacionados com a retenção nos cuidados de VIH, permitindo identificar grupos e áreas prioritárias de intervenção. Os resultados obtidos são concretizados no final em recomendações concretas ao governo e administração central, aos serviços de saúde e às organizações comunitárias.

The growing migratory flow and the increase in new HIV diagnoses among immigrants in Portugal make the issue of access to healthcare for this population increasingly relevant, with implications for their own health and for public health. Although our legislation promotes universal access to health, this right is influenced by many other factors (determinants) that are important to know in order to promote the development of truly integrative policies and practices. With this objective, this research was carried out using a mixed qualitative and quantitative research methodology: 1) focus groups (with healthcare professionals and NGO collaborators), and 2) questionnaire survey, applied nationally to 407 immigrants with HIV resident in Portugal. The results found allowed us to identify a greater vulnerability of immigrants with HIV compared to nationals, revealing that more than half have already experienced various obstacles in accessing and using health services. The identified determinants largely coincide with those referred to in the literature for immigrants in general, including factors inherent to individuals, to healthcare professionals and their relationship with users, and structural issues of the national healthcare service. It was also found that the factors that influence access to and use of general health services do not fully coincide with those related to retention in HIV care, allowing the identification of priority groups and areas for intervention. The results obtained are ultimately translated into concrete recommendations to the government and central administration, healthcare services and community organizations.
