A prescrição de genéricos em Portugal: Perspectiva dos farmacêuticos das farmácias comunitárias da ARS de Lisboa e Vale do Tejo

Abstract

Poucas questões são tão controversas na área da saúde como a introdução de genéricos no mercado de medicamentos. Desde a introdução deste tipo de medicamento no mercado português, muitas questões foram levantadas contra e a favor destes medicamentos, muito se especula em relação à sua qualidade e eficácia. No entanto, é uma realidade que estes têm forte influência na redução dos gastos do Estado na área da saúde. Existem diversos intervenientes no processo de prescrição – aconselhamento na farmácia – dispensa/aquisição do medicamento, e o farmacêutico é aquele que de uma forma ou de outra contacta mais directamente com todos os outros intervenientes. Pelo que, constituindo um importante influenciador, torna-se necessário entender a sua perspectiva. O presente estudo foca a perspectiva dos farmacêuticos no que diz respeito aos medicamentos genéricos, e para tal foi utilizado um inquérito por questionário como ferramenta de recolha de dados para entender estas mesmas questões. Verificou-se, de uma forma muito resumida, que do ponto de vista do farmacêutico, o médico evita colocar nas suas mãos a decisão de substituição do medicamento prescrito por um genérico, devido ao receio de alguma perda de poder junto dos utentes. Constatou-se também que os utentes sentem-se ainda pouco esclarecidos relativamente às vantagens dos medicamentos genéricos e que como tal têm algumas dúvidas quanto ao seu preço e efeito. O farmacêutico considera que deveria ter maior poder e autoridade no processo. Sendo que as medidas a tomar para aumentar a confiança nos medicamentos genéricos, quer por parte dos médicos quer dos utentes deverá passar por uma maior formação e informação, respectivamente. Os resultados obtidos com esta investigação poderão ser importantes para desenvolver estratégias futuras por parte das autoridades de saúde, no domínio das políticas do medicamento, bem como de empresas do sector, ou para eventuais estudos futuros sobre o tema.

In the health care area, few questions are so controversial as the introduction of generics medication in the medicines market. Since there was the introduction of this kind of medicines in the Portuguese market, many questions have taken place against or on behalf of these medicines, existing many speculation concerning its quality and efficiency. However, it is real that they have a strong influence in the reduction of the expenses by state in the health care system. There are several actors in the prescription process – pharmacy advisement - release / acquisition of medicine and, despite the way, the pharmacist is the one that, contacts more directly with the remaining actors. Being an important influence, it becomes necessary to understand its opinion. The present study, focus the pharmacist perspective regarding the generics medication, using a questionnaire as a tool to understand these questions. In summary, trough the perspective of the pharmacists, there was verified that the physicians avoid to put in their hands the decision of substitute the prescribed medicine by a generic, fearing the lost of power in front of its patients. Another conclusion was that the patients feel few elucidated about the advantages of generic medication and consequently have some doubts concerning price and effect. The pharmacists consider they should have more power and authority in the process. In addition, the measures taken to increase the confidence in the generics medication, by physicians and also by patients, should be based on more formation and information, respectively. The obtained results with this investigation could be important to develop future strategies by the companies of the sector, or for further eventual studies about this item.
