Introducing change at the Hong Kong University-Shenzhen Hospital: an empirical study

Abstract

The consultation model in Chinese hospitals is based on the specialist system that originated from the Soviet Union. Under this model, patients will directly consult a specialist without previous scanning by a generalist doctor. Considering the rapid and successful development of general practice in European countries, the Chinese government was enlightened to introduce and establish a similar consultation model in China. This thesis investigates the process of change carried out to introduce this new mode of “Primary Care by General Practitioners and Secondary Care by Specialists” in the newly established Hong Kong University – Shenzhen Hospital (HKU-SZH), aiming to uncover the reasons for resistance and for the difficult growth in the number of outpatients wishing to adopt the new model. Firstly, the thesis systematically reviews the background of general practice, its development in China and current status at both home and abroad. Secondly, theories of organizational change and business reengineering as well as their application in health care reform were reviewed. Thirdly, the changing process of HKU-SZH was introduced. HKU-SZH, newly built in 2012, took the lead to adopt the consultation model “Primary Care by General Practitioners and Secondary Care by Specialists” in China as an important practice in the country’s health care reform. The field survey was conducted through two different questionnaires administered to patients and medical staff survey. A total of 1120 responses were successfully received from patients and 224 from medical staff. Results show that there are two main reasons for the non- increase of the outpatient number in general practice department of HKU-SZH: (1) patients’ perception that general practice will not provide the same level of care as specialists’; and (2) medical staff’s, especially nurses and technicians, lack of understanding of the benefits of the planned change. Therefore the hospital is suggested to enhance public recognition towards general practice by strengthening the promotion and providing convenient services. It should also strengthen the training of its medical staff enhancing their technical level, self-confidence and insight of general practice, in order to reduce resistance during the organizational reform.

Importado da antiga União Soviética, o modelo de consulta nos hospitais chineses é baseado na consulta a médicos especialistas segundo o qual os pacientes consultam diretamente especialistas sem recurso prévio a médicos generalistas. Considerando o que se passa em países europeus, o governo chinês decidiu introduzir um novo modelo de consulta em que o paciente deverá primeiro visitar um médico generalista que eventualmente o poderá conduzir a especialistas se necessário. Esta tese investiga o processo de mudança ocorrido no recém estabelecido Hong Kong University – Shenzhen Hospital (HKU-SZH) com o fim de identificar as causas para a resistência à mudança e a dificuldade deste hospital em conseguir fazer crescer o número de pacientes que adotam o novo modelo “generalista primeiro, especialista depois”. A tese faz uma revisão sistemática da literatura referente à prática generalista, ao seu desenvolvimento na China e à situação atual no país e no estrangeiro. Em seguida a tese revê as teorias da mudança e reengenharia organizacionais com aplicação específica à reforma de sistemas de saúde. Por fim a tese analisa o processo de mudança no HKU-SZH. Estabelecido em 2012, este hospital foi um dos primeiros na China a adotar o novo modelo de consulta “primeiro generalista, depois especialista” como uma prática importante no contexto da reforma do sistema de saúde do país. Para o trabalho de campo utilizaram-se dois questionários, um para os pacientes e outro para o pessoal de saúde. Foi recebido um total de 1120 respostas válidas de pacientes e 224 do pessoal de saúde. Os resultados revelam que existem duas razões principais para o não aumento do número de pacientes que adotam a nova prática: (1) a percepção dos pacientes é a de que os médicos generalistas não fornecem o mesmo nível ou a mesma qualidade de cuidados de saúde que os especialistas; e (2) o pessoal de saúde, em especial enfermeiros e técnicos, tem dificuldade em compreender e aceitar os benefícios da mudança. Sugere-se portanto que o hospital realize campanhas junto do público esclarecendo-o sobre o verdadeiro papel dos médicos generalistas e as vantagens do novo modelo. Deve também conceber programas de formação que esclareçam o seu pessoal por forma a diminuir a resistência à mudança.
