The overall aim of this thesis was to identify the critical risk and protective factors associated with successful outcomes in Therapeutic Residential Care (TRC). This thesis was composed of five interconnected studies. Studies 1 (quantitative) and 2 (qualitative) were systematic reviews. The first study introduced a dual conceptualisation of non-specific (e.g., sex and abuse and trauma history) and specific treatment factors (e.g., treatment model and placement decision making) associated with lower psychopathology. The latter study identified quality indicators in TRC directly from the voices of young people (e.g., trusting relationships, family involvement and, relationship continuity). Studies 3–5 provide empirical evidence from TRC facilities in Portugal using a cross-sectional design. Study 3 (mixed methods) examined young people’s lived experiences of TRC and psychopathology. Study 4 (qualitative) examined the lived experiences of staff only. Study 5 (quantitative) identified clusters of young people based on the dual-factor model of mental health, and non-specific and specific treatment factors. Across studies, key findings highlighted the importance of maltreatment history as a critical non-specific treatment factor associated with risk, whereas the Milieu Factor (trusting relationships with staff, peers, family, and the community) emerged as a pivotal specific treatment factor for both risk and protection. These findings culminate in the first bottom-up framework to improve outcomes in TRC providing actionable, evidence-based recommendations for policy and clinical practice.
