Os factores críticos na cirurgia de ambulatório

Abstract

Ao longo das últimas duas décadas a Cirurgia de Ambulatório tem vindo a expandirse, sobretudo em países desenvolvidos e, desde o início deste século, o seu crescimento em Portugal foi notável. Em 2008, mais de 50% de todas as intervenções cirúrgicas programadas foram efectuadas neste regime. A realização de cirurgia em regime de ambulatório deve centrar-se no objectivo maior de proporcionar aos utentes uma atenção de qualidade, tanto nos aspectos técnicos como a nível psicossocial, fazendo-o a custos adequados (cost-effective quality care) e tendo o doente como protagonista (patient centred program). Neste contexto, torna-se fundamental avaliar a satisfação dos utentes e levar os profissionais de saúde a avaliar as suas práticas, comprometendo-se em projectos de mudança. Foi aplicado um questionário, desenvolvido tendo por base o Modelo ServQual, a 60 doentes de uma Unidade de Cirurgia Ambulatória. Em relação à satisfação global com a Unidade, 56,7% dos utentes inquiridos atribuiu o valor máximo numa escala de dez pontos de satisfação, e 96,7% atribuiu um valor superior ou igual a sete. Sobre a realização das suas expectativas, 55,0% dos respondentes atribui a classificação máxima, considerando que a sua experiência na Unidade foi “muito superior ao esperado”, e 71,7% atribui também a classificação máxima à probabilidade de escolher de novo este serviço. Aos profissionais de saúde intervenientes no processo foi aplicada uma entrevista utilizando a técnica de focus group. Os profissionais envolvidos no processo consideram que é no Pré-Operatório e no Pós-Operatório que podem ocorrer o maior número de falhas.

Over the past two decades, Ambulatory Surgery has been expanding worldwide, especially in developed countries, and since the beginning of this century its growth in Portugal has been remarkable: in 2008, more than 50% of all elective surgeries were performed in this setting. Ambulatory Surgery should be focused on providing its users a greater attention to quality, not only with regard to technical aspects but also at the psychosocial level, and on doing so with appropriate costs (cost-effective quality care) and placing the patient as the central character (patient centred program). In this context, assessing the user’s satisfaction is essential, as well as leading health professionals to evaluate their practices and engage in projects of change. A questionnaire based on the ServQual Model was applied to 60 patients in an Ambulatory Surgery Unit. Regarding global satisfaction, the Ambulatory Surgery Unit received the highest classification in a ten-point scale from 56.7% of the patients, and received seven or more points from 96.7% of all patients. Regarding the fulfilment of expectations, 55.0% of the patients classified it as the highest possible, considering their experience in the Unit “much better than the expected”. 71.7% of the patients classified the probability that they would choose this Unit again in the future as the highest possible. A focus group interview was applied to the health professionals involved in the process, who considered that the pre-operative and post-operative periods are the ones in which a higher number of faults may occur.
